Please complete this certificate and return a copy to the AFT/Share My Lesson’s approved CTLE Sponsor along with your PD certificate. You must retain your copies for eight years. It is not necessary to send a copy of this form to the Office of Teaching Initiatives unless it is requested in the event of an audit or for use in obtaining an Initial Reissuance. A separate form must be completed for each training.

NYSED CTLE Information
As of 2016, the State Education Department requires Continuing Teacher and Leader Education (CTLE) for all holders of professional classroom teaching, educational leadership, or Level III teaching assistant certificates. Details on exact rules and how to track your hours can be found on the SED website.
